All Out Sports Ball Waiver Form PARTICIPATION AGREEMENT FOR ALL OUT SPORTS BALL PROGRAMS 1.CONDUCT: I/My participating child hereby agree to abide by and support the current All Out Sports Ball rules of play, personal conduct and Covid 19 regulations. II.USE OF IMAGE: I hereby grant All Out Sports Ball the right to use, at their sole discretion any photographs of or about myself for publicity, advertising or other promotion of All Out Sports Ball. I understand that this may include written, pictorial, or video materials. III.LIABILITY WAIVER: 1.1 I agree as a precondition of my participation in the program as organized by All Out Sports Ball and in further consideration of their inviting me and/or registering me to do so, to be strictly bound by the terms of this waiver. 2.1 I acknowledge that participation in the program as conducted in accordance with its laws and regulations may involve inherent risks that can cause serious injury to its participants, particularly to its players. 3.1 I fully understand the risks and dangers associated with my participation in the program as conducted in accordance with its laws and regulations and do accept the same entirely at my own risk. NOTE: I have read and understand this agreement. I accept that by registering with All Out Sports Ball and signing this document in the space provided below, that I understand this document is also considered a liability waiver. I, the undersigned agree to abide by such rules, regulations and decisions of all recognized jurisdictional bodies within All Out Sports Ball itself and its Executive.
